Wyoming Community Health Centers
Community Health Centers across the state provide health care to everyone, regardless of insurance status or ability to pay. The Wyoming Primary Care Association supports and promotes Community Health Centers so they may continue to provide high quality, integrated comprehensive care including medical, dental, pharmaceuticals, and behavioral health.



Ag Worker Health & Services
Get the care you deserve, uniquely tailored to the needs of agricultural workers and their families, including primary and family care, dental, and behavioral health.

Health Care for the Homeless
We provide evidence-based primary medical care and mental health services, as well as medication dispensary and care management, to individuals and families in Natrona County who are either homeless or at-risk of homelessness.

Crossroads Healthcare Clinic
Funded through Health Resources and Services Administration (HRSA), Crossroads Healthcare Clinic provides primary medical care, mental health services, and substance abuse services to individuals and families who are uninsured, under-insured or insured on a service for fee sliding scale. We also participate in Medical Assisted Treatment Program (MAT Program) for treatment of opioid addiction.

Educational Health Centers of Wyoming
The Educational Health Center of Wyoming meets the health and wellness needs of our communities by providing accessible, high-quality care while educating health professionals, in collaboration with the University of Wyoming Family Medicine Residency program.

Community Health Centers of Central Wyoming
Our care teams practice a model of care that puts your well-being at the center of everything we do. By treating illness and promoting wellness, our clinical teams build strong relationships with their patients to improve care coordination and management, leading to better outcomes, greater satisfaction, and lower costs.

One Health
One Health provides integrated medical and behavioral health care; dental; pharmacy; substance use disorder treatment; community outreach and education; and obstetrics. We refer to our model of integrated health care as “Whole Community Care,” striving to provide healing at the individual and community level through compassionate and holistic primary care.

HealthWorks
In addition to primary medical care for all ages, we provide mental and behavioral health services and affordable prescriptions—all from one convenient location. To make health care affordable, we offer a sliding scale fee structure for our income-eligible patients. We also accept most insurance.

Health West
Health West is a non-profit, federally funded community health center that offers medical, dental, pediatrics, women’s health, pharmacy and behavioral health services. We empower our patients by proactively providing quality, affordable, patient-centered health care.
